For starters, our doctor, Dr. JW Chang, is a pediatric dentist. That means that after finishing dental school, he completed an additional 2-year residency program in pediatric dentistry. Instead of just treating his patients like little adults, he is sensitive to the unique dental issues that kids face and how best to work with them so they feel calm, safe, and comfortable. Our office also has been specifically designed to be a fun, welcoming place for children with our prize toy machine, video games, cavity-free club program, and more.

You are more than welcome to fill out these forms ahead of time by clicking the button below. Just don’t forget to bring out the completed forms to your child’s first appointment! If you’d rather complete them in our office, we ask that you please arrive 10 to 15 minutes early so we can make sure to keep your scheduled appointment time.
